I have been taking Anafranil for OCD for many years and it keeps that under control. When I started having more difficulty with walking etc. I found myself having trouble dealing with my worsening condition and my GP started me on Paxil CR. I have come across a forum about withdrawal from this medicine, that it is a living hell. Dr. Heller’s Answer: The simplest way to get off Paxil is to switch to Prozac for two weeks, then stop the Prozac. Prozac leaves the body slowly and avoids the withdrawal effects from Paxil. The other SSRI medications leave the system quickly and can cause side effects. Prozac is different. This is a previous answer Ive given for this question… What are the symptoms of SSRI withdrawal? 1) dizziness 2) light-headedness 3) vertigo or feeling faint 4) shock-like sensations 5) paresthesias 6) anxiety 7) diarrhea 8) fatigue 9) gait instability 10) headache 11) insomnia 12) irritability 13) nausea or vomiting 14) tremor 15) visual disturbances Symptoms began within 1-7 days after stopping an SSRI (sometimes with a reduction in dose), if SSRI used for a month. Approximately 50% are better in a week, for others the symptoms last much longer. Prozac leaves the body slowly and is not associated with SSRI withdrawal. Slowly tapering the SSRI is one way to avoid this phenomenon, as is a temporary switch to Prozac. Question: I can’t believe you classify ADD/ADHD as disorder that somehow, over time, can turn into BPD. ADD has to do with brain structure – as it is developed invitro, it is not a personality disorder. Dr. Heller’s Answer: ADHD doesn’t ‘turn into’ BPD. ADHD is in no way a personality disorder. It’s mostly genetic and in my opinion isn’t a disease, but a genetic trait that has advantages in some environments and disadvantages in others. Those with ADHD are hunters in an accountants’ world. They tend to be brilliant, clever, and creative. They are frequently charming as well. The correlation of adolescent and adult ADHD with BPD is approximately 50/50. Untreated and undertreated ADHD can result is a great deal of emotional pain which can cause the BPD. There was a study in Italy last year that showed this as well. Being called lazy, stupid, airhead, etc., literally causes brain damage. So can trying hard and not succeeding despite obvious intelligence (usually proven in IQ tests). A recent study showed there is no difference in the brain between emotional and physical pain. It seems ever since he has gotten on it he has been moody a lot. There is a lot of stress in his life. He is sick from work, no money coming in, people not leaving us alone, 5 kids to raise. Well, anyway about 2 weeks ago he snapped..and did things that I have never seen him do. But he took a extra dose of Zoloft that day. He is on 150 but he could not remember if he took it and took another dose of 150. I need to know if the Zoloft can make a person NOT like them self because something was not right that night. We had a good day and things seemed good. Please if u can help. Thanks. Dr. Heller’s Answer: In and of itself its highly unlikely Zoloft would do that. If he has the cognitive GAD (generalized anxiety disorder) combined with the BPD (borderline personality disorder) it could make him worse like all the SSRIs. BuSpar can prevent this problem. Anyone can “snap” under enough stress, so its possible the problem has nothing to do with the medication. Question: Dr. Heller: When I read the definition of what BPD is, I wanted to tell you that our daughter (now 23) displayed the symptoms when she was between the ages of 3-4. She still displays the same symptoms. Was officially diagnosed at age 14 after being in psych. hospital for 11 days in ’85. No groups to help: hosp. put her on 5 different medicines at same time; sent her home for us to cope and wished us well: very sad statement. Weaned her off drugs and went from there. She is aware of being “different” but tries to be very responsible for her actions. Tried vitamin therapy by infusion and started to respond, but then ran off to get married at 18. So now her husband tries to deal with her and it’s really rough: do not know how this will end up, but I keep reading and researching. Hopefully, I can reason with her to listen. Years before, after finding out diagnosis, stepped back to take a good look at my husband and could finally have answers to why he acts the way he does: grew up in Brooklyn N.Y. No wonder so crazed out talk about upheaval supreme. Anyway, just wanted to pass thoughts on: if suicide can be detected in children at an early age, why not BPD? Where were you 12 years ago when I needed you? Dr. Heller’s Answer: A severe childhood depression with psychotic features can mimic BPD. The most likely explanation was childhood onset bipolar disorder (http://pks.947.myftpupload.com/BiPlrKid.htm) . Bipolar is likely a form of epilepsy as is the BPD. Its extremely common for individuals with childhood onset bipolar to also have BPD once puberty starts. It does appear very likely that the BPD doesnt begin until puberty. The use of multiple medications is not unusual, nor is it a sign of poor treatment.
