WHAT CAN BE DONE FOR PAXIL INDUCED ERECTION PROBLEMS?
Question: Dr Heller, I have only been on Paxil 20 mg a day for the past month. I am feeling wonderful, but wonder if I need to remain on the medication. I was diagnosed with a mild case of clinical depression and this is what my Doctor prescribed. I have been experiencing erectile dysfunction since taking the Paxil and wondered if getting off it now after a month will either return the depression, cause any of the side effects that I’ve read about, or have some adverse affect. I am a healthy, 40 y/o white male in good physical shape. I take no other medications and have no other significant medical history. Your reply would be greatly appreciated. Dr. Heller’s Answer: In general treating for only one month is a bad idea. SSRI medications like Paxil usually don’t cause erectile problems, usually a difficulty having orgasms. You need to speak with your physician about trying an alternative antidepressant and possibly checking your testosterone level, since low testosterone can cause depression.

WHAT CAN BE DONE FOR MY SONS MARIJUANA ADDICTION?
Question: Hello, my name is Cathy and I have a 20 year old son who has been struggling with marijuana addiction. He is in college and has many negative encounters associated with this drug, although he fails to see the necessity of quitting. He has tried in the past to no avail, and he is presently seeing the school counselor, but has missed several appoints due to various reasons. He says he is on a plan to slowly cut down and then quit, but I am very apprehensive about this, probably because we have been struggling with this for nearing 8 years. I have tried my best, but do realize it is up to him. He says that his mind races and that this calms him down, that he gets anxious when he doesn’t smoke pot and that he is in fact self medicating. What steps can we take to help him? Please respond. I don’t know what more to do! Dr. Heller’s Answer: You identified the problem – hes self medicating his anxiety disorder(s). Marijuana “helps” many anxious people feel better, at a price. He needs to get his anxiety problems diagnosed and treated. WHAT ARE THE EFFECTS OF ALCOHOL ON DEPRESSION AND BPD?
Question: I would like to ask you about the effect(s), if any, of alcohol (even in moderate/small amounts) on people that have BPD and depression. In particular, this individual is taking Effexor. I can’t find much information on this particular issue. Thank you. Dr. Heller’s Answer: Alcohol is a depressant and can make depression worse – especially increasing the risk of suicide. Alcohol is a problem for the BPD in three ways: 1) It seems to neutralize the effects of Prozac, so the unprovoked mood swings and chronic anger come back. 2) It reduces frontal lobe control of behavior, so increased anger combined with reduced behavior control can result in disaster. 3) Since dysphoria (anxiety, rage, depression and despair) is an epileptic phenomenon, an alcohol withdrawal dysphoria seizure can occur 6-72 hours after the alcohol was consumed. The individual with BPD doesnt relate the two glasses of wine on Friday night with the rage or depression on Sunday, but they are strongly related. As far as I know theres no additional risk to the individual taking Effexor when combined with alcohol.

WHAT ARE THE EFFECTS ON BPD WITH PREGNANCY, GENETICS AND GETTING OLDER?
Question: Hi, I am looking for information, esp. original research on two questions: — BPD and pregnancy: I know that few careful studies have been done on how BPD affects pregnancy. But how does pregnancy, e.g. pregnancy hormones, affect BPD? – I have heard of some beneficial influences, but only in anecdotal contexts… — inheritable BPD: Aside from the older observations of clustering of BPD (or really only BPD symptoms) in families, are there any newer studies on genetic components?– BPD history: It is quite striking how much more optimistic (and politically correct) the newer literature is, from the NIMH, books and websites, compared to that only a decade ago. One striking example is that it is often observed that BPD gets better in the fourth decade of life, but what is the latest on the reasons for this? It appears that the older literature, (along with many therapists in the trenches) seems to discount the phenomenon as just the patients wearing themselves out… Thank you for any references, info or tips. Dr. Heller’s Answer: 1) There’s very little in the literature on pregnancy in the BPD. I’ve seen it go both ways. Some women with BPD do great during pregnancy, better than usual. Many have a very, very hard time. I’ve worked with some women who had to take Tegretol during the pregnancy because the suicide risk was so great. Most are very anxious to get back on medication after the pregnancy (and/or breast-feeding) are over. 2) There have been some studies about heritability, but like most topics pertaining to the BPD, scientific data is sparse. The BPD clearly can run in families. Some of the most fascinating cases I’ve seen are adoptions where the BPD risk is high in the biological offspring that weren’t raised by the biological parents. Ive rarely seen the other way around. I think the ADHD genetic link is more important than the BPD link as a cause of BPD. 3) The “improvement” over time is usually limited to the ability to maintain a job. Most individuals with BPD during adolescence still fit criteria and experience the medical BPD symptoms throughout adult life – continuing to old age. Interpersonal relationships generally remain a disaster. Self-mutilation usually lessens or ceases after adolescence is over. The worst symptoms are during adolescence when the limbic system goes into hyperdrive. Maturity and growing past adolescence make a huge difference, but dont make the core medical symptoms go away. Having successful family and love relationships are to me far more important than work success, and without treatment most people with BPD have a difficult time throughout their lives with these relationships.

Question: Dear Dr. Heller, My name is Tom, I have recently come to a point of reviewing myself. I realize I may be a compulsive liar. I tend to tell a situation that really happened to me but I pretty much push all my faults & responsibility on to others. By doing this I begin to lie about things, things like I’m always right I did this and that where I did everything opposite. For example, Just recently I got fired from my job & I guess it was due to my loss of control over my temper. So in my termination letter, said I was terminated due to failure to meet requirements and failure to improve my performance. But when my family & friends ask me, I go on telling them I was fired because of my work ethics, I went on & said I was being too nice of a guy & my place didn’t like that but the truth is I’m just a total Jerk to my patients. Honestly, I never liked working at the place I was at. I had so much stress & that built up to anger & hate towards my job. Well how can I stop this compulsive lying disorder? I’m so need in help. please help.. Hopeless Tom Dr. Heller’s Answer: Sometimes anxiety disorders, particularly with ADHD, can cause lying. Compulsive lying may be a sign of OCD. I’d recommend going over the screening test at http://pks.947.myftpupload.com/screen.htm and get any diagnoses treated. Its obvious your self esteem is in bad shape, and needs help. Counseling is a good idea. I’d also highly recommend listening to Zig Ziglar’s “How to Stay Motivated” tape series.

IS PROZAC CAUSING MY HEADACHES?
Question: First of all, thanks for providing this service– reading through your website has been a real eye opener! I am a 27 year old female diagnosed with depression at about 16 years of age. I have been on several different antidepressants and am currently on 40 mg Prozac once a day for about 8 months. Two months ago, I began getting four to six headaches a week that no over the counter pain medication can seem to handle. I saw my doctor for this and he diagnosed the headaches as tension/migraine in origin, and I have been on a few different prescriptions for them. None seem to help. I was wondering if the Prozac could be the cause of the headaches? Can you please shed some light on this? By the way, my blood pressure is 90/63, so I know I don’t have high blood pressure. THANKS SO MUCH! Dr. Heller’s Answer: If you had been on Prozac for 6 months and it didnt cause headaches, its highly unlikely that Prozac is contributing to the headaches. The most common causes of your problem would be BPD dysphoria (anxiety, rage, depression and despair), sinus infections, posture problem, TMJ dysfunction (including grinding your teeth), and stress. Treatment options depend upon the correct diagnoses.
