Question: Hello Doctor,I am very curious as to why some antipsychotic drugs, particularly Risperdal, cause weight gain. Is it because they lower the patient’s metabolism, or is it because they cause an increased appetite?Thank you for taking the time to answer my question. Dr. Heller’s Answer: They appear to cause insulin resistance – the same phenomenon that eventually causes diabetes. Some authorities recommend adding metformin (a diabetes medication) to reduce the risk of weight loss and development of diabetes.

Question: What is the treatment for an adult with ADHD who has had heart by-pass surgery?Our son was just diagnosed with ADHD and we think that my husband may have it as well. Dr. Heller’s Answer: Chronic is when it persists over time, the exact length of time is individualized.Dysphoria (anxiety, rage, depression and despair) is a symptom borderlines experience under stress or when they perceive a threat. It appears to be an epilepsy phenomenon (nerve cells firing inappropriately and out of control). Question: Dear Dr. Heller,I am a professional and a graduate from Marquette University. Due to an injury several years ago I have undergone numerous methods of treatment, but to no avail. Several months ago I had major back surgery, the fusion. In the past several years I have been off and on narcotics, 4x a day.This February, one year from my surgery, my pain management doctor has discussed with me getting off the narcotics. I do believe I have build up a tolerance level because the 4 narcotics I take does not seem to help a great deal. In addition, I have a cage in place and one bone is not healing properly and I am wearing a bone simulator, hoping to help heal the bone graph. I do have great concerns getting off the pain medication because I believe it allows me to function on a day to day basis.I have 4 children, 3 under the age of 7. I also work part time and my husband is overseas, so I don’t have much help available. What is your opinion regarding the medication?From,Uncertain! Dr. Heller’s Answer: I see many patients with similar complaints. They virtually all have posture problems so the back and neck never heal right and cause chronic pain. By putting on a posture support in the office most of the pain can be stopped right away. Its like having the alignment off on your car and treating the vibration pain in your arms with narcotics – the alignment needs to be fixed.I frequently prescribe a posture support to be worn a minimum of 4 one hour periods daily. I usually refer to a good physical therapist for posture straightening and whatever assistance is necessary. Sometimes one leg is shorter than the other and a lift can make a huge difference.While chiropractors can help, Ive had better results with osteopathic manipulation technique and craniosacral therapy. These techniques are done by osteopaths (D.O.s) who specialize in manipulation of the entire musculoskeletal system including ribs, the pelvis, etc. I get this performed regularly on me.
