Question: Please tell me about “Fractured Enjoyment.” Is this actually a disorder in the DSM-IV-TR? Thanks! Dr. Heller’s Answer: No it isn’t. As I wrote in “Biological Unhappiness”, it’s not an official diagnosis, but it is clearly a real clinical entity. It’s more a less a combination of dysthymic disorder and the research criteria for the unofficial diagnosis of “Depressive Personality Disorder.” It was written about in “Listening to Prozac” by Dr. Peter Kramer. It’s apparently a genetic syndrome where the person exists between depression/dysthymia and normal. They tend to not understand why others find it easy to be happy and they don’t. There’s a great deal of rejection sensitivity and frequently negativism. The cartoon characters Eeyore and Charlie Brown describe the concept. In the 1920’s Dr. Emil Kraepelin described it as the “depressed personality.” The term has never caught on. I became aware of the syndrome when Prozac became available as it’s the only medication that seems to work for this genetic depression. People switched from other antidepressants to Prozac (fluoxetine) feel dramatically better in 3 days when they have this syndrome.
